Addendum:

2026 Palestinian Festival – Vendor Giveaway Data Collection Effective August 20th 2026, in conjunction with the Westend Dental Clinic Privacy Policy (rev. June 3, 2025).

A. What This Addendum Covers

This addendum applies only to personal information collected at the Westend Dental Clinic booth during the 2026 Palestinian Festival at Ottawa. It supplements the main Privacy Policy above. Where the two conflict, this addendum governs festival-collected data.

B. Information Collected at the Festival

For the purpose of the on-site and online giveaway, we may collect:

  • First and last name

  • Phone number (mobile, for SMS winner notification)

  • Email address (optional)

  • Self-declared status: festival-goer or vendor

  • Consent checkmarks (giveaway entry; optional marketing follow-up)

No health, dental, insurance, or treatment information is collected at the festival booth or through the giveaway form. Festival data is stored separately from our dental-management system and is not linked to any patient record.

 

C. Purpose

Festival Collected data is used solely to:

  • Run, verify, and announce the giveaway

  • Contact winners to claim prizes

  • If the participant checked the marketing box:

    • Send follow-up emails, messages, or phone calls

  • Retain records for 180 days post-festival in case of a dispute
     

D. How It's Collected & Stored

Entries are captured via paper sign-up sheet on-site or forms submitted on [www.westenddentalclinic.com].

E. Sharing

Festival data is not sold or rented. It may be shared with:

  • The Palestinian Festival organising committee, to the extent they host or co-run the raffle platform
     

F. Calls & Marketing Consent

By checking the consent box, the participant authorises Westend Dental Clinic to send up to two [2] [SMS / email messages or phone calls]  regarding patient outreach.

G. Your Rights (Festival Data)

The rights in §7 of the main policy apply. In addition, festival participants may request deletion of their entry within 30 days of the event by contacting the privacy officer below. Requests are honoured within 10 business days.
